- Surgical management of presbyopia represent the current major challenge in refractive surgery. Many techniques have been developed meaning that none has been able so far to restore perfectly the accommodation and to be proposed to all presbyopic patients. Actually the majority of methods compensate the loss of accommodation and have the additional goal of improving the near vision with respect of distance vision and no significant loss in quality of vision. The different corneal (photoablation, intrastromal femtosecond, inlays) and intraocular (multifocal and accommodative IOLs) procedures will be reviewed and discussed in terms of concept, interest, indications, limitations and outcomes. The discussion should lead to a decisional tree according to the age and the ametropia. Monovision has still a place when future treatment are oriented to the perspective of a true restoration based on lenticuloplasty with femtosecond and new accommodative lenses.
