Objective/Purpose
To evaluate the safety and efficacy of 1.8mm micro phacoemulsification (1.8mm microphaco) in grade 4 + cataract , a leap in further reduction in incision size

Summary of Content
In this video author has shown that consuming grade 4+ nucleus with reduce bore of needle (1.8mm microphaco ) is as effective and safe as other incisional size phacoemulsification .The main concern while doing 1.8mm phacoemulsification is reduced hold , chopping difficulty , anterior chamber fluctuation and more time consuming . With the avaibility of ventury system in phacoemulsification machine , slight change in chopping technique i.e chopping in peripheral part of bag ,vertical chopping and increasing the height of bottle effectively cuts almost all grade of cataract with same speed and safety .During the study mean phacoemulsification time , effective phaco time (EPT) , corneal status , anterior chamber depth and cells flare, visual outcome and stabilisation of refraction were noted .
