VI-12 Management of Postoperative Occlusio Pupillae after Cataract Surgery with Underlying Posterior Capsular Plaque
Objective/Purpose
To demonstrate an effective technique to handle postoperative occlusio pupillae after cataract surgery and removal of the underlying posterior capsular plaque. Summary of Content This video demonstrates a case of a 7 year old boy who had undergone cataract surgery elsewhere followed by chronic uveitis resulting in formation of an iris tissue band occluding the pupillary area and leading to formation of occlusio pupillae. Central iris tissue covered over and adherent to the PCIOL was dissected and removed using vitreoretinal scissors and forcep.Pupillotomy was performed.Iris retractors were used to stretch the pupil and view the underlying posterior capsular plaque behind the PCIOL.Removal of the underlying posterior plaque was done with the help of modified cystitome and vitreoretinal scissors through the opening of a previous iridectomy.Vitreoretinal scissors were used to cut the resistant posterior plaque which was followed by anterior vitrectomy to remove the remaining remnants of the plaque and create a round central posterior capsulotomy .The surgical technique resulted in a clear central visual axis which was followed by removal of the four iris hooks. This video illustrates that difficult situations can be tackled by meticulous surgical approach and effective postoperative management with appropriate postoperative control of inflammation by steroids leads to increased visual outcome.
