Development of a Direct Ophthalmoscope Enable to Test Vision Fixation Using Characters and Figures

As we considered that an easily-operated and low-cost device for evaluating and training vision fixation state and preferred retinal locus (PRL) was necessary due to the importance of instructing patients on how to use their eyes in low-vision rehabilitation, we developed an ophthalmoscope incorporating characters and figures. Issues during this development process were glare and recording of the examination. Therefore, in this time we mounted an absorptive filter to a remodeled ophthalmoscope and set a recording device.

Twenty low-vision patients, aged between 12 and 70, included 15 patients with optic atrophy, cone dystrophy or age-related macular dystrophy who had central scotoma in the eye and 5 patients with concentric stenosis due to retinitis pigmentosa. Their visual acuity was between 0.03 and 0.6.

We integrated characters and figures into the remodeled ophthalmoscope (BXα), tested them to 20 low-vision patients using an absorptive filter (CCP-UG), and investigated the results. At the same time, we fixated a smartphone (iPhone) and made recordings.

Results and Conclusion
Integrating an absorptive filter into a remodeled ophthalmoscope able to test vision fixation with characters and figures resulted in decreased photophobia in all patients. While reducing the burdens on patients, the absorptive filter also for better contrast, thus improving visibility. Moreover, fixating the smartphone as a recording device made steady imaging possible in addition to recording moving images and sound.
Practicality of the ophthalmoscope was improved by overcoming the issue of subject photophobia in the remodeled ophthalmoscope with the use of an absorptive filter and recording by fixating a smartphone. Furthermore, as using figure targets of images such as pandas could make vision fixation testing of pediatric low-vision patients possible, and the use of a smartphone could allow this ophthalmoscope to be utilized in remote area medical care, its range of use may be set to expand in the future.
