Validation of the conventional resection criteria in patients with hepatocellular carcinoma in terms of incidence of posthepatectomy liver failure and long-term prognosis

The aim was to clarify the validity of the current indication in terms of incidence of posthepatectomy liver failure and long-term prognosis in patients with HCC. A retrospective cohort study was performed using 265 patients who underwent major hepatectomy. Makuuchi criteria and ICGKrem>0.05 criteria were validated. Among 265 patients, 107 (40.4%) and 158 (59.6%) patients were within/beyond Makuuchi criteria, respectively. No statistical difference of DFS (within vs. beyond, 11M vs. 15M) or OS (29M vs. 40M) was observed. On the other hand, 223 (84.2%) and 42 patients (15.8%) were within/beyond ICGKrem>0.05 criteria. No statistical difference of DFS (13M vs. 14M) or OS (36M vs. 35M). Makuuchi criteria and ICGKrem>0.05 criteria were not associated with risk of recurrence and life expectancy.
