Development of an assessment scale for the training of laparoscopic distal gastrectomy using Delphi methodology

[Background] With the wide spreading of minimally invasive surgery (MIS), the importance of basic skill training for MIS has been recognized. For a past decade in Western countries, there has been more effort to create specific assessment tools, such as laparoscopic cholecystectomy, hernia repair and colectomy. However, the assessment scale for the skills evaluation of laparoscopic distal gastrectomy (LDG) has not been developed due to the more complex procedure and small case number in Western countries. We conducted the cognitive task analysis (CTA) to describe important steps in LDG procedure, then started to determine LDG expert consensus regarding essential points required on the assessment scale for the performance of LDG using the Delphi method. The aim of this presentation is to demonstrate the methodology and process of CTA of LDG and development of the assessment scale for the LDG using Delphi method. [Methods] 1st step - CTA: The knowledge and the criteria, which required completing each aspect of LDG was enumerated on the basis of text book, instructional video and the interview from the novices and experts. 2nd step - Delphi method: Development of the assessment scale is in progress using the Delphi method to converge the opinions with online survey by anonymous of 20 experts (endoscopic surgical skill qualification system: qualified surgeon). [Summary and Future direction] We demonstrate the methodology and process of the development of the assessment scale for the skill for LDG, and also indicate the direction of the LDG training program using the assessment scale.
