CIHHV-6 was activated in a patient with X-linked severe combined immunodeficiency

Objective:Chromosomally integrated human herpesvirus-6(ciHHV-6) is largely considered as non-pathogenic condition affecting about 1% of the population. Recently, it was reported that active viral replication from ciHHV-6 can be induced in vitro. However, its pathogenicity is still controversial including whether ciHHV-6 is reactivated or not in vivo.We herein report ciHHV-6A reactivation in a patient with X-linked severe combined immunodeficiency.
Methods:We performed quantitative real-time reverse transcription PCR(qRT-PCR), immunohistochemical and virus-isolation assays as well as sequence analysis to elucidate that the replicated and the integrated HHV6 were the same strains.
Results:The patient was suffered with hemophagocytic lymphohistiocytosis at 3 month of age. HHV-6 IE antigen was stained positive in the bone marrow sample and HHV-6 RNA was detected in his Peripheral blood (PB). We found that he and his father had ciHHV6A. Patient's PB was cultured with unrelated cord blood (CB), which resulted in giant cell formation. We detected the viral RNA and viral antigens in the infected CB cells in vitro. We also detected the viral RNA in the PB, when he suffered with thrombotic microangiopathy during engraftment period of stem cell transplantation but it disappeared after engraftment of the donor cells. The strains of HHV-6 from the genome of the patient, his father and the infected cord blood cells were found to be the same by partial sequence.
Conclusions:CiHHV6A can be activated in vivo, especially in immunocompromised host.
