Graft rejection remains a significant problem following CBT. A few studies have shown a role of cellular immunity on graft rejection. Although statistical analysis of clinical data has revealed that presence of donor-specific anti-HLA antibodies (DSA) were associated with graft rejection, direct effects of humoral immunity on graft rejection have never been demonstrated in CBT. Here, we show a coordination of donor-specific cytotoxic T lymphocyte (CTL) and DSA in a case of cord blood allograft rejection. Two CTL clones, CD4-CD8+ and CD4+CD8-, were isolated from the recipient blood at the time of graft rejection. Both the CTL clones were of recipient origin by short tandem repeat analysis. The CD4-CD8+ and CD4+CD8- CTL clones recognized mismatched B*54:01 and DRB1*15:02 molecules as an alloantigen, respectively, which were expressed in donor cells but not in recipient cells. The CD4-CD8+ and CD4+CD8- CTL clones inhibited colony formation (CFU-GM) of unrelated bone marrow mononuclear cells (BMMNCs) positive for B*54:01 and DRB1*15:02, respectively, suggesting that the CTLs impaired hematopoietic stem cell (HSC) engraftment. DSA against HLA-B54 was detected in pretransplant recipient serum by Luminex assay. ADCC activities were evaluated using unrelated BMMNCs as targets, and recipient serum and NK cells as effectors. The DSA inhibited colony formation of unrelated BMMNCs positive for B*54:01, suggesting that the DSA impaired HSC engraftment. Taken together, these data demonstrate that a coordination of cellular and humoral immunity in graft rejection following CBT.
