Immunoregulatory mechanisms of blocking IL-21 signaling in early post-transplant period of acute graft-versus-host disease model

Interleukin (IL)-21 is a key role in the development of T helper cell (Th) 17-derived cytokine that aggravates acute graft-versus-host diseases (GVHD) after allogeneic bone marrow transplantation (allo-BMT). We investigated the immunoregulatory mechanisms of blocking IL-21 signaling in early post-BMT of acute GVHD model. IL-21 blockade enhanced Treg and Th2 cell differentiation, while inhibiting Th1- and Th17-derived transcription factors and cytokines as a modulator of activated T cells. In addition, IL-21-/- cell recipients had decreased B220+IgD-IgM- memory B and B220+IgD-CD138+ plasma cell. Effector B cells decreased including B220+CD23+CD21- follicular B and B220+GL-7+Fas+ germinal center formation, while B220+CD23-CD21+ marginal zone B, which act as regulatory B cells increased during IL-21 blockade. We observed decreased levels of B cell activating factor (BAFF) which regulates B and T cells during acute GVHD and confirmed that this regulation was through BAFF-receptor (BAFF-R) as TNF receptor-associated factor 3 (TRAF3) was reduced compared to other BAFF signals. Therefore, this study showed that IL-21 blockade attenuates acute GVHD directly via the dual regulation of T and B cell homeostasis and indirectly via BAFF pathway-mediated inhibition.
