Clinicopathological characteristics of MGRS and MGUS with renal dysfunction in a single institution

Background: Leung N et al suggest MGRS (Monoclonal gammopathy of renal significance) as a part of MGUS associated with renal dysfunction, and MGRS shows a causal relationship between renal damage and monoclonal gammopathy like monoclonal immunoglobulin deposition disease. MGRS should be considered early diagnosis and treatment to preservation and restoration of renal function. In order to detect MGRS earlier, we confirmed the clinicopathological characteristics of MGRS and MGUS with renal dysfunction. Methods:We analyzed clinical data and pathological findings in a renal biopsy obtained from 21 patients with monoclonal gammopathy including 10 MGRS and 11 MGUS in our hospital from 2010 to 2012, excluding myeloma kidney. Results: MGRS demonstrated significantly larger amount of proteinuria and more number of urinary M pretein-positive cases as compared to MGUS. However, there were no significant differences in the renal function, the amount of M protein and the percentage of plasma cells in bone marrow. In CD38 gating, CD19-negative cells were increased in MGRS. In renal pathology, MGRS showed significant tubulointerstitial damage. Conclusions: Because of the increase of CD19-negative cells showing the presence of monoclonal proliferation of malignant plasma cell, we think that MGRS tends to progress to myeloma and renal failure faster as showing tubulointerstitial damage than MGUS. Therefore, in the case of MGUS showing large amount of proteinuria and/or urinary M protein, further examinations including renal biopsy should be performed for the early diagnosis and treatment.
