10 cases of HIV-associated lymphoma

HIV infection increases the risk of developing not only AIDS-defining malignancies including non-Hodgkin lymphoma (NHL) and primary CNS lymphoma (PCNSL), but also non-AIDS-defining malignancies including Hodgkin lymphoma (HL). We report 10 cases of HIV-associated lymphoma treated in our hospital from 2002 to 2012. Median age was 38 years [24-61 years], and all patients were male. The histological diagnoses were DLBCL in 6 patients, AITL, ALCL, HL, in 1 patient, respectively. One patient was diagnosed with PCNSL based on both imaging features and the presence of EBV DNA in CSF by PCR without a brain biopsy. Median CD4 T-cell count at lymphoma diagnosis except one case of leukemic forms of AITL was 50 /μL [1-211 /μL]. In 6 patients, lymphoma occurred under ART, whereas in 4 patients, HIV infection and lymphoma were diagnosed concurrently and ART were initiated immediately. All of 8 patients except PCNSL were in advanced stages, and 6 patients had extranodal lesions. NHL patients were treated with CHOP±rituximab therapy, HL patient was treated with ABVd therapy, and PCNSL patients were treated with whole brain radiotherapy. All of 8 patients who received chemotherapy experienced grade 3-4 adverse events (grade 4 neutropenia in 7, febrile neutropenia in 4, infection in 3, mucositis in 3). 7 patients achieved CR or PR, whereas 3 patients exhibited SD or PD, and 6 patients are alive. These results suggest that the characteristic of HIV-associated lymphoma are more aggressive than those of non-HIV-associated lymphoma.
