Composite lymphoma with in situ mantle cell lymphoma and follicular lymphoma

Composite lymphoma(CL) including mantle cell lymphoma(MCL) is rare. MCS, especially in situ type, is more likely to be associated with follicular lymphoma(FL). Here we report a 56-year-old man with CL which composed of MCL and FL. He was diagnosed as grade 3 FL with normal-size mantle zone by the biopsy of his right brachial tumor. Simultaneously, advanced stage MCL with diffuse growth pattern of sigmoid colon tumor and abnormal lymphoid cells in bone marrow were detected. Thereafter, the right brachial tumor was re-examined and its mantle zone cells were immunophenotypically positive for cyclinD1(CCND1) and cytogenetically positive for IgH-CCND1 fusion gene. Consequently, he was diagnosed for CL with FL and in situ MCL. Considering of the poor prognosis of MCL, it might be important to detect the existence of in situ MCL in FL lesion, at early stage.
