Clinical study of mantle cell lymphoma involving gastrointestinal tract

Objective: Gastrointestinal (GI) tract involvement of mantle cell lymphoma (MCL) presents in a variety of forms, ranging from multiple lymphomatous polyposis (MLP) to a slight mucosal change. In this study, we determined the clinic features of patients with GI tract involvement of MCL.Patients and Methods: The clinical features of four MCL patients in our hospital, whose gastrointestinal lesions were followed endoscopically, were retrospectively analyzed.Results: Studies have reported that gastrointestinal MCL occurs most commonly in the stomach and intestines. However, in this study, MCL involved the esophagus in two patients (Cases 1 and 2) and almost the entire gastrointestinal tract from the stomach to rectum in one patient (Case 3). In one patient (Case 4), GI tract involvement of MCL was diagnosed only through biopsy. Slight intestinal mucosal changes present at the time of recurrence evolved into overt MLP in one patient (Case 2) who had failed to respond to a variety of chemotherapy regimens. This may indicate that lymphoma cells have diffusely infiltrated the lamina propria to submucosa when MLP is not grossly evident, and the MLP becomes apparent over time. Conclusion: GI involvement of MCL presents in a variety of forms, and, in some patients, endoscopic findings reflect the clinical course of MCL involvement of the GI tract.
