Graft versus ATLL effect for relapse after allogeneic stem cell transplantation

[Background] In recent years, allogeneic stem cell transplantation for adult T-cell leukemia/lymphoma(ATLL), highly aggressive hematological neoplasm, has become feasible treatment with long-year overall survival. We experienced highly effective Graft versus ATLL effect for relapsed ATLL patients after allogeneic stem cell transplantation.[Patients and methods] we present 8 cased of relapsed ATLL patients after allogeneic stem cell transplantation treated in our institution from April 2002 to March 2013. We analyzed them with clinical status at transplantation, conditioning regimens, stem cell sources, treatment options after relapse, and long-term outcomes.[Results and Discussion] Median age of relapsed patients is 51.2 ages(33-61 ages), and clinical status at transplantation were 7 in PR, one in CR. One patient received myeloablative conditioning, 7 patients received non-myeloablative conditioning. Stem cell sources were 4 from related peripheral blood stem cell, 3 from unrelated bone marrow, 1 from related bone marrow. Median time of relapse after transplantation were 13 months. Treatments after relapse were only weaning of immunosuppressant in 2, weaning of immunosuppressant and donor lymphocyte infusion in 3, donor lymphocyte infusion in 2, chemotherapy and radiation in one. 5 of 8 patients were alive and free from relapse, but one patients died from TMA, 2 patients died from the ATLL progression. Graft versus ATLL effect is thought to be highly effective for relapse after allogeneic stem cell transplantation.
