An unusual case of B-cell acute lymphoblastic leukemia (B-ALL) with an indolent course

Background: B-cell acute lymphoblastic leukemia (B-ALL) is a neoplasm of precursor cells committed to the B-cell lineage. The patients with this disease present with consequences of cytopenia and need chemotherapy immediately.Case: A 56-year-old woman, who presented with leukocytopenia, visited our department in September 2011.Laboratory tests showed: WBC 2000/μl (neutro: 24.6%, lymph: 74.4%, atypical lymph: 5%), Hb: 11.4g/dl, Plt: 24.6×104/μl. We performed follow-up once every 3 months, and her data remained stable. In January 2013, about one year after her first visit, we performed bone marrow aspiration. It revealed hypercellular marrow with about 80% immature lymphoid cells. The blast cells were positive for terminal deoxynucleotidyl transferase (TdT), CD10 and CD19, but negative for CD5, 20 and CD23. Clonal cytogenetic abnormalities were not seen. Autoimmune antibody was not detected and chest and abdominal CT showed no evidence of lymphoadenopathy or hepatosplenomegaly. A diagnosis of B-ALL was made based on the bone marrow findings. However, due to there being no clinical symptoms and normal blood cell counts except for neutrophils, we have observed the patient without treatment. The laboratory data have remained stable. Conclusion: This is a rare case of B-ALL showing an indolent clinical course. To our knowledge, no similar cases were previously reported in the literature.
