Marked eosinophilia caused by interleukin-5-producing cardiac myxoma

There are only a few case reports of eosinophilia associated with interleukin-5 (IL5)-producing tumors. Here, we describe a case of marked eosinophilia caused by cardiac myxoma producing IL5. A 72-year-old man was referred to our hospital because of marked eosinophilia. He had a 40 years' history of asthma and then mild eosinophilia since his 40s. Two years prior to admission, his white blood cell (WBC) count increased to 27,400/μL (Eo 73.7%), and on admission, up to 59,980/μL (Eo 86.7%). He had a low-grade fever and fatigue but no abnormal findings on physical examination. Bone marrow was hyperplastic by a cluster of mature eosinophils without increase in immature blast cells. Cytogenetic study showed normal karyotype and no evidence for FIP1L1/PDGFRα and TEL/PDGFRβ. Infectious episodes including helminthes were also ruled out. The echocardiography revealed endocardiac mass attached to the mitral valve. The serum IL5 level was elevated to 44 pg/mL (ref. <4 pg/mL), suggesting that cardiac tumor may cause eosinophilia via IL5 secretion. Considering the potential risk for tumor embolism, he was referred to another hospital for tumor resection. Soon after the cardio-surgery, eosinophilia was dramatically improved with concomitant decrease in the serum IL5 level (5 pg/mL). The tumor was pathologically diagnosed as cardiac myxoma and positive for IL5 mRNA by RT-PCR analysis. To our knowledge, this is the first report of eosinophilia caused by cardiac myxoma producing IL5.
