Evaluation of peripheral WT1 mRNA as a biomarker for prognosis in patients with MDS

[Purpose] This study investigated the clinical value of monitoring the peripheral WT1 mRNA (WT1) levels in myelodysplastic syndromes (MDS) patients. [Patients and Methods] We examined the expression of WT1 by RT-PCR in 456 samples from 133 patients with MDS (2patients with RARS, 20 with RCUD, 22with RCMD, 17 with RAEB-1, 17 with RAEB-2) and any other bone marrow (BM) disorders (4 with AA , 1 with PRCA, 11 with secondary cytopenia, 9 with MDS/MPD, 29 with secondary AML evolved from MDS). All of them were diagnosed at our institute from 2008 to 2012. WT1 levels were compared with those in BM blasts, and monitored with clinical course in some patients. [Results] WT1 levels (threshold >50 copies/μg RNA) were very low in patients with AA, PRCA, RCUD, secondary cytopenia, whereas those were generally high in AML and correlated with the evolution of MDS subtypes. Therefore, we found a significant correlation between WT1 levels at diagnosis and the IPSS, WPSS score. MDS patients were divided into the three groups by WT1 levels (low; WT1 less than 50copies/μg, intermediate; 505000copies/μg). 4-year survival rates depended on the degree of those categories(low 57%, intermediate 21%, high 0%). However, the expression of WT1 in MDS was not correlated with the IPPS-R score because WT1 expression could not clearly discriminate the patients with cytogenetic abnormalities defined by IPSS-R. [Conclusions] WT1 mRNA in PB might be an useful prognostic marker to guide management of MDS, especially for the disease progression caused by proliferation of BM blasts.
