Azacitidine induced pneumonitis - a case report

(Introduction) Azacitidine (AZA), the first hypometylating agent for myelodysplastic syndrome (MDS), has been available since 2011 in Japan. There were few repots of AZA-induced lung injury. We report a case of MDS complicated by pneumonitis possibly induced by AZA(Case report)A 85-years-old Japanese male was referred to our clinic for further examination of anemia pointed out accidentally, he was diagnosed MDS ( RAEB-2 ) and was treated with AZA for seven days. Seventeen days after the end of administration, he noted fever and was treated with broad- spectrum antibiotics after two taps of blood culture, which was sterile. Biomarkers of fungal, Pneumocystis jiverocii and cytomegalovirus infection was also negative. Antibiotics was not effective, CT of the chest was performed and revealed interstitial pneumonitis of the right lung. Corticosteroid was administered and, several weeks after, pneumonitis resolved completely(Discussion)Pneumonitis induced by AZA is a rare condition and mechanism is unclear. Also it is unknown whether hypometylating effects of AZA affect the process. In some reports, corticosteroids are effective to AZA-induced pneumonitis.
