Bone marrow imaging of humeral and femoral marrow by multi-detector computed tomography in MDS

Background: Myelodysplastic syndrome (MDS) is a heterogeneous group of bone marrow failure disorder. Here, we analyzed the relationship between the abnormal medullary pattern in appendicular skeleton (AS) by low-dose multi-detector CT (MDCT) with laboratory variables, overall survival (OS) and leukemia-free survival (LFS) in MDS patients.Patients: We performed MDCT of humerus and femurs in 57 untreated MDS patients between 03/2009 and 02/2013. MDCT patterns were categorized as fatty, nodular or diffuse patterns. Retrospective review of clinical and laboratory features was performed. OS and LFS were analyzed by the Kaplan-Meier method. Univariate and multivariate analysis were used to assess the effects of prognostic factor. Results: Among the 57 patients, 13 had fatty patterns, 27 had nodular patterns and 17 had diffuse patterns. Patients with diffuse patterns on MDCT had a significantly lower Hb level and shorter LFS and OS compared to those with fatty or nodular patterns (P=0.011, P<0.01, P=0.029). LFS and OS of 17 patients with diffuse patterns were significantly shorter than that of the 40 patients with fatty and nodular patterns (P <0.01, P=0.029). Univariate analysis showed that RAEB, low Hb level (<8.0 g/dl), chromosome abnormality and diffuse patterns on MDCT had negative prognostic impacts on LFS and OS. On multivariable analysis, diffuse patterns on MDCT were emerged as independently negative prognostic predictors for LFS.Conclusions: This study showed that MDCT imaging of AS provided important information for the diagnosis and prognosis of patients with MDS.
