Treatment results for elderly patients with acute myeloid leukemia in a single institution

Background: The prognosis of elderly patients with acute myeloid leukemia (AML) has been reported to be poor. Patients and method: Thirty-five patients age 65 years or older diagnosed at our institution as having AML or AML with myelodysplastic related changes (AML-MRC) between 1996 and 2011 were analyzed retrospectively. Results: 11 patients (31 %) were with AML-MRC or acute panmyelosis with myelofibrosis, defined as the adverse risk group. In the remaining patients, cytogenetically defined risk groups were favorable in 8 patients (23 %: t(15;17) in 4 and t(8;21) in 4), intermediate in 15 (43 %), and unknown in 1 (3 %). For patients with t(8;21), intensive chemotherapy (ICT) containing anthracycline and various doses of cytarabine achieved complete remission (CR) in 3 (all aged over 70, 1 over 80) of 4 (75 %) with relapse rate (RR) of 33 %. For patients with t(15;17), ICT in combination with all-trans retinoic acid (ATRA) achieved CR in 3 (1 aged over 70) of 3 (100 %) without relapse, and ATRA alone in 1 (aged 92) of 1 (100 %), who relapsed. For patients of the intermediate risk group, ICT achieved CR in 5 (3 aged over 70 and 1 over 80) of 9 patients (56 %) with RR of 80 %. For patients of the adverse risk group, ICT achieved CR in 3 (none aged over 70) of 7 patients (43 %) with RR of 67 %. 3-year overall survival rate in the favorable, intermediate, adverse risk groups were 75 %, 17 %, and 9 %, respectively. Conclusion: Our results were compatible with the previous reports. Since RR in the intermediate risk group was quite high, the newer therapeutic approach is required.
