Retrospective risk-stratified outcome analysis of 14 cases of APL treated at our hospital

The prognosis of patients with acute promyelocytic leukemia (APL) has dramatically improved with the combination of all-trans retinoic acid (ATRA) and anthracycline based chemotherapy, which is considered as the standard treatment for APL. We retrospectively analyzed 14 patients who were diagnosed with de novo APL at our institution between April 2001 and March 2013. The median age was 54 years (range 19 to 71 years). Male to female ratio was 1:1. Two were stratified as low risk according to the LPA99 study, while nine as intermediate risk and two as high risk. All patients were treated with LPA96, LPA99, or LPA2005 protocol, in which induction therapy consists of ATRA with idarubicin mono-chemotherapy, followed by 3 cycles of consolidation therapy modified for each risk groups. The median follow up time was 4.6 years (range 0.4 to 9.8 years). All patients achieved CR after induction therapy and subsequent molecular CR. No severe infections but one epidural hemorrhage were seen during consolidation therapy. Five received maintenance therapy for 2 years. Three (including two high-risk patients) relapsed after less than 1 year in CR, including two cases with CNS infiltration. All of them underwent stem cell transplantation and two are alive. The 5-year overall survival rate was 78% (range 37 to 94%), and the 5-year relapse free survival rate was 76% (range 42 to 91%). The induction therapy resulted in a good outcome with 100% CR rate. However, early relapse and CNS infiltration in some cases suggest the need for improved treatment strategy for high-risk patients.
