Three case reports of secondary AML following S-1 adjuvant chemotherapy for gastric cancers

(Introduction) S-1(tegafur, gimeracil, osteracil) is a pyrimidine anti-metabolite widely used in Japan for gastric cancers (GC). Second malignancy including therapy-related leukemia (TRL) by S-1 is reported to be rare compared to other compounds such as alkylating agents and topoisomerase II inhibitors. We hereby report 3 cases of TRL following S-1. (Results) Case 1; 68-year-old man who had been prescribed S-1 for a year after distal gastrectomy (DG) in May 2007 was diagnosed as acute myelocytic leukemia (AML), M2 (46,XY) in Oct. 2011. Successful induction led him to CR, being maintained to this day. Case 2; 71-year-old man who had DG in Apr. 2010 took S-1 for 6 months afterwards, but was obliged to stop due to adverse effects. In Dec. 2011, he developed AML, M2 (inv16). CR has been achieved without relapse. But recurrence of GC was referred in Jan. 2013, which is now being treated. Case 3; 82-year-old man took S-1 for 12 months after DG in May 2010. Due to a recurrence, he restarted S-1. 6 months later, diagnosis of AML, M0 (complex) was obtained. He is now treated with best supportive care. (Conclusion) As with our 3 cases, overall survival for solid tumors significantly improved with the advance of chemotherapy, suggesting higher incidence of TRL. Careful monitoring should be taken not only to alkylating agents and topoisomerase-II inhibitors administered patients but also to those with S-1. Moreover, despite the assumption of TRL having poor prognosis, our cases indicate that some type of TRL responds well to conventional treatment for AML and should be fully treated.
