INTRODUCTION: Iron overload causes various organ damages. When transferrin (Tf) is fully saturated, non-transferrin-bound iron (NTBI) appears in the serum. Since NTBI relates to oxidant stress, monitoring serum NTBI levels is crucial for iron-overloaded patients. Several methods for NTBI determination have been reported, but complicated procedures were required. Therefore, we aimed to establish a novel assay system utilizing conventional automated analyzers. METHODS: Serum NTBI is initially chelated by NTA and iron is then transferred to Nitroso-PSAP which had chromogenic effect when bound to iron. This system needs no pre-treatments of serum samples, and the data analyses can be performed by a conventional automated analyzer. We examined the basic performance of this novel reagent: lower detection limit, relationship to the Tf saturation, and the correlation of values between our system and the gold standard HPLC method. We also determined the influence of deferasirox (DFX) on our system. RESULTS & DISCUSSION: Lower detection limit of our system was 0.34 µM. High NTBI values were shown in serum of high Tf saturation. The correlation coefficient for NTBI values between our system and HPLC method was 0.923, indicating that the accuracy of our system is comparable to the gold standard method. We also showed that DFX in the sample had no influence on the values, suggesting that our system can be used routinely for monitoring NTBI even if on the DFX treatment. Thus, our novel NTBI assay system should be a powerful tool for studying physiological and clinical importance of NTBI.
