Clinical usefulness of MRI-based liver iron concentration measurement in iron overload patients

Background:Iron overload (IO) resulting from multiple RBC transfusions (TFs) induces free radicals and it leads to DNA damage and organ failure. Iron chelation therapy (ICT) using deferasirox has been established in TF-dependent IO patients. Serum ferritin level is currently used as a marker for IO, however, it is influenced by other causes and exact estimation method for IO has not yet been established. In addition, it is an unsolved problem whether ICT is necessary for patients who have a history of multiple TFs, but currently TF-independent. Patients and Methods: MRI-based liver iron concentration (MR-LIC) was measured according to the protocol by Gandon et al (Lancet 2004). Briefly, various gradient-recalled-echo sequences of the liver were obtained with a 1.5T MRI, and MR-LIC was estimated applying MRI signal intensity of liver and muscle to the formula described by Gandon. Patients who are currently TF-dependent (n=20) or have a history of RBC TFs more than 8 units before 3 months (n=14) were included. Results: The median MR-LIC and serum ferritin value in patients who are currently TF-dependent was 195±50mol/g (5-340mol/g) and 1084ng/ml (312-12190ng/ml), respectively. MR-LIC significantly correlated with serum ferritin value in patients without inflammatory complications or bleeding. However, it did not correlate with serum ferritin in patients with these complications. In patients who have a history of RBC TFs, five patients showed high MR-LIC (>100mol/g).Conclusion: MR-LIC measurement is a useful method for estimating IO in TF-dependent patients.
