Etiological spectrum and prognoses of hyperferritinemia at Kanazawa Medical University Hospital

Introduction: Hyperferritinemia may be caused by many diseases and conditions; i.e. red blood cell transfusion, malignancies, infections, liver diseases, rheumatoid diseases and organ failures. The aims of this study were to assess the etiological spectrum and prognoses of hyperferritinemia at Kanazawa Medical University Hospital (800-bed university hospital).Patients and methods: We retrospectively analyzed the medical records with hyperferritinemia (serum ferritin level higher than 1,000 ng/mL) at Kanazawa Medical University Hospital between March 2011 and February 2012.Results: One-hundred fifty-six patients (66 women and 90 men; median age: 68, range 0-90 years old) were recruited. The median serum ferritin level was 1,477 ng/mL (range; 1,000-14,782 ng/mL). The principal etiologies were malignancies (28.8%), red blood cell transfusion (23.1%), infections (16.7%), liver diseases (11.5%) rheumatoid diseases (5.1%) and others. Fifty-four patients died during follow up period. Especially, the prognosis of patients with hyperferritinemia caused by malignancies was poor; twenty seven of 45 patients died.Conclusion: The three major etiology of hyperferritinemia were malignancies, red blood cell transfusion and infections at our hospital. The prognosis of patients with hyperferritinemia was poor, especially, caused by malignancies.
