Study of blood β2 microglobulin level in patients with end stage renal disease on hemodialysis with either High Flux or Low Flux membrane

Objectives: To study blood β2 microglobulin(β2 m) level in patients with end stage renal disease on hemodialysis with either high flux(HF) or low flux(LF) membrane.
Methods and Patients: A cross – sectional study was conducted on 30 patients in Cu Chi Hospital ( dialyzed by HF Nipro dialyzers with β2 m clearance coefficient of 0.88) and 56 patients in Binh Dan Hospital (dialyzed by LF Nipro dialyzers with β2 m clearance coefficient of 0.36) from January 1st to June 30th, 2010. All patients aged > 15 years and were dialyzed 4-hours/session, thrice a week, with dialyzers of 1.5 surface, bicarbonate dialysate, and blood flow in the range of 200-250 ml/min. The β 2m value in blood of these two arms were compared to each other to see whether the high flux membrane was better than the low flux regarding the elimination of this globulin. Both groups were well matched for duration on dialysis, BMI, gender, age, hemoglobine...
Results: The level of blood β 2m in both groups were higher than normal with lower value in the HF group compared to the LF group (26631,460 ± 7511,38 µg/L vs 51580,11 ± 18811,09 µg/L, P <0,01). Also, the level of blood urea was higher than normal in both group with lower value in the HF group compared to the LF group (pre-dialysis: 21,77 ± 6,67 mmol/L vs 26,54 ± 7,64 mmol/L, P= 0.005, and post-dialysis: 7,06 ± 2,54 mmol/L vs 9,41 ± 3,26 mmol/L, P = 0.001, respectively). The Kt/V value was low in both groups (1,14 ± 0,23 in HF group and 1,07 ± 0,46 in LF group, P > 0,05). There was a lineaire correlation between the level of blood β2m and the level of pre-dialysis blood urea (r = 0,264, P < 0,05).
Conclusions: HF dialyzer appeared to be better than LF dialyzer in elimination of middle molecular weight substances
