Spread Progression of Thromboses in Arterio-Venous Fistula Diagnosed by Color Doppler Ultrasound in Hemodialysis Patients, and Treated Results

【Purpose】 Discovering early spread progression of thromboses at arterio-venous fistula (AVF) in hemodialysis pts.  【Material and method】 From January 2005 to December 2010, 75AVF/67pts: 29men and 46women, average aged: 52.1 years, average hemodialysis times: 1 year 2 months, suspected to have thromboses, examined the vascular color Doppler ultrasound by Voluson 730 ProV ultrasound machine to detect thromboses in the AVF.  【Result】 75AVF/67 pts were blocked: 3 AVF/67 pts were blocked before AVF no completely. 72AVF/67 pts were blocked after the AVF (not blocked completely=24 AVF/34 pts, blocked completely =27AVF/27pts). Treated results: Blocked completely: 24/75 AVF treated by Lovenox 1mg/kg/day * 2 time/day * 3-15 days, recovered the blood flow with 2th ultrasound examining, having 19/24 AVF cleared thromboses completely and 5/24 AVF not recovered. Not blocked completely: 40/75 AVF treated, 2th ultrasound examining had been cleared thromboses, recovered the blood flow (28/40 AVF blocked completely and 12/40 AVF blocked no completely). Total: 51/75 AVF recovered the blood flow, having complete thromboses after hemodialysis 6h00, discovered spread thromboses completely from 12cm to 30 cm length after hemodialysis 24h00, by pts were treated in time and adequate Lovenox dose. 24/75 AVF were not recovered because pts discovered and treated too late. The dose & the time of Lovenox treatment are inadequate. 69/75 AVF were blocked mainly in the summer. 64/75 thrombi pts started at the AVF and 11/75 thrombi pts started far from the fistula about 7-20cm.  【Conclusion】 Color Doppler ultrasound detected exactly early spread progression of AVF thromboses in hemodialysis pts.
